Appointment Scheduling:
If you would like to schedule an appointment for treatment with Dr. Mantha, please fill out the quick form below with your information. Our office coordinator will reach out to you promptly. For immediate assistance, during normal business hours, please call our office at 909-377-2939.

New Patients:
Initial consultation with Dr. Mantha is required for all new patients. The doctor will take the time to understand your concerns and answer your questions, so that you find the best treatment plan to meet your needs.

New patient consultation appointments require a pre-appointment charge of $25 to your preferred credit card. If you choose to follow through with the treatment plan, this amount will be discounted from your first treatment appointment. Our office coordinator will take your billing information over the phone.

Financing Options:
Verve partners with Care Credit if you need help financing your treatment.
